To Err is Human: Case Application of Error Management and Prevention

Presentation Description / Summary: Medical errors are common in both human and veterinary hospitals and outpatient settings with drug errors being the most common type of error. This session will utilize case examples of medication-related errors to illustrate the concepts of appropriate error management in the moment as well as post-error evaluation to prevent future occurrences of similar errors. Participants will also learn how to apply error management and prevention strategies in situations where multiple facilities/professions are involved, such as errors that involve community pharmacies. Strategies will be presented in a way that connects them to creating a strong medication safety culture within the veterinary team and improving interprofessional relationships with other medical health professionals.
